German wholesale terms 2026: HSA, KSA and Rx cash discounts under review

The cash-discount debate is a reason to recalculate the complete supply offer. What matters to pharmacies is the purchasing benefit remaining after fees, financing and contractual adjustments.

Why review your German pharmacy supply agreement now?

The debate about new cash-discount options has renewed attention on purchasing agreements. Earlier payment is not automatically worthwhile: the actual benefit depends on the eligible invoice base, contractual conditions and financing costs. This article concerns Germany and a commercial assessment method, not a binding interpretation of an individual contract.

Understanding Handelsspannenausgleich (HSA) and Konditionssicherungsausgleich (KSA)

Offers become comparable only when each adjustment has a defined calculation base. For Handelsspannenausgleich (HSA), a trade-margin adjustment, examine the reference margin, assessment period, thresholds and deductions together. For Konditionssicherungsausgleich (KSA), a terms-protection adjustment, establish the trigger, formula and settlement process. These English descriptions explain the German terms; the individual contract determines their meaning and effect.

The comparison should show eligible turnover, excluded products, credits and subsequent charges alongside the headline percentage. A nominally stronger offer may yield less once the accounting period is settled. Neither HSA nor KSA alone guarantees additional income.

Rx cash discounts, payment deadlines and value dates

For prescription medicines (Rx), assess the discount rate, discount period, normal payment deadline and value date together. Check high-cost medicines, allocation-controlled products and exclusions. Over-the-counter medicines (OTC) and non-medicinal retail ranges may have different terms.

Illustrative calculation, not a market quote: a 1% discount on an eligible invoice of €100,000 produces €1,000. Financing the remaining €99,000 for 20 days at 8% annually costs about €440 using a 360-day year. About €560 remains before other fees and effects. Verify that the payment really is brought forward by 20 days and identify the funds required.

Check charges and reconcile the actual result

Delivery-round charges, returns fees, controlled-drug charges, cooperative membership costs and service fees affect the result. Service remuneration or payment for data reporting should be included only where the work, entitlement and payment conditions are clearly agreed.

A monthly procurement review should reconcile the supply offer with invoices, credits and charges. Main-supplier commitments, minimum purchasing levels and availability also matter. Attractive prices may be offset by the extra work of repeatedly sourcing missing products.

FAQ: practical pharmacy questions

Is the highest cash-discount rate always the best offer?

No. Compare the eligible base, exclusions, payment timing, financing costs and other deductions to establish the benefit actually retained.

Which documents support a wholesale-terms review?

The current agreement and schedules, representative invoices, credits and charges, plus payment deadlines and product-group information provide a useful basis.
